Population Health Management
Local Health Departments (LHDs) have made several advances in gathering clinical data electronically. This detailed data can be processed to generate micro and macro level reports to analyze health outcomes among various groups to determine health patterns. LHD report data can be compared with other LHDs to get a better understanding of outreach programs, policies and procedures. This comparative data aids in improving patient outcomes and reducing health care costs. Data plotted on GIS provides important overview of health outcomes within different groups, including socio-economic factors, food deserts, etc.
Session Objectives:
- Describe the measures collected by public health departments
- Explain how metrics are correlated to improve health outcomes.
Come join us and learn more about today’s Population Health Management. We hope to see you there.
